Early Intervention in Schizophrenia: The Role of Adolescent Brain Development and Genetic-Environmental Risk Interactions

Schizophrenia is a complex psychiatric disorder that often emerges during adolescence, a critical period of brain development marked by significant structural and functional changes. Changes in the adolescent brain, particularly in regions like the prefrontal cortex and hippocampus, interact with genetic predispositions and environmental stressors to increase the risk of schizophrenia. By examining prior work in this space, this review demonstrates that excessive synaptic pruning, altered dopamine signaling, disrupted connectivity, and epigenetic modifications can interact with risk genes and life experiences, increasing the development of schizophrenia. Current diagnostic gaps in capturing the pre-psychotic phase are from an overreliance on retrospective data and a lack of a holistic analysis. Diagnostics and treatments mostly focus on people who already have psychosis, rather than those in the early stages before psychosis develops, which limits the opportunities for prevention during the critical pre-psychotic phase. Additionally, much of the existing research relies on asking patients about pre-illness behaviors, which may lead to inaccurate or incomplete reporting of early experiences. The main objective of this paper is to present how the complex interplay between adolescent brain development and genetic-environmental interactions contribute to schizophrenia vulnerability and explore how these insights can guide earlier, more personalized intervention and treatment strategies for at-risk youth.
